Consent to receive informational and promotional messages

With this consent, freely, of my own free will and in my own interest, I give the consent of EF GEF LLC (TIN 7801737783 / OGRN 1247800113120 / KPP 780101001 Address: 199406, City of St. Petersburg, ext. Municipal Okrug Gavan, Maly V.O. Ave., building 64, building 1, building 1, room 100-N) (hereinafter referred to as the “Operator”) to receive information and advertising messages from the Operator (hereinafter referred to as the “Newsletter”) using telecommunication networks/information and telecommunication networks, including the Internet, in accordance with the requirements of the legislation of the Russian Federation on personal data and advertising.

1. Mailing channels:

1.1. electronic mail (e-mail);

1.2. SMS messages;

1.3. telephone calls and/or voice messages.

2. Personal data used for the Newsletter:

2.1. last name, first name, patronymic (if available);

2.2. telephone number;

2.3. email address.

3. Mailing Purposes:

3.1. sending information messages about the work of the clinic (for example, appointment confirmation, visit reminders, schedule changes, organizational notifications);

3.2. sending advertising/marketing messages about special offers, promotions, news, events and services of the Operator.

4. Procedure and conditions for processing personal data for the purposes of the Newsletter:

4.1. The Operator processes personal data for the purposes of the Newsletter using automation tools and without the use of such tools.

4.2. The list of actions (operations) includes: collection, booking, systematization, accumulation, storage, clarification (updating, changing), extraction, use, depersonalization, blocking, deletion, destruction, as well as transfer (provision, access) to persons carrying out processing on behalf of the Operator (for example, communication/mailing service providers) subject to the conclusion of contracts and compliance by such persons with the requirements of the legislation of the Russian Federation.

5. Cross-border transfer of personal data is not carried out by the Operator.

6. Duration of consent:

6.1. This consent is valid from the moment of putting a mark (tick) in the “I agree to receive the newsletter” field on the website and/or signing this document, as well as other actions that clearly indicate consent, and is valid until the consent is withdrawn or until the termination of the Operator’s activities, unless otherwise provided by the legislation of the Russian Federation. If the Newsletter is not carried out for a long time, the Operator has the right to stop processing data for the purposes of the Newsletter after 3 (three) years from the date of the last interaction with the subject, unless otherwise required by law.

7. Opting out of the Mailing list and withdrawing consent:

7.1. I am notified that I can at any time refuse the Mailing List and/or revoke this consent by sending the Operator a notification to the email address hello@fgfmed.ru marked “Unsubscribe” or “Revoke consent to mailing list”, as well as in another way specified in the message (for example, with the “Unsubscribe” link, if provided).

7.2. The Operator stops sending the Newsletter within a reasonable time after receiving such notification and stops processing personal data for the purposes of the Newsletter, unless there are other legal grounds for processing.

8. Confirmations of the subject of personal data:

8.1. I have read the Privacy and Personal Data Processing Policy posted on the website https://fgfmed.ru

8.2. It was explained to me that consent to the Newsletter is voluntary and does not affect the ability to send a request/make an appointment;

8.3. I was explained the rights of the personal data subject, including the right to withdraw consent at any time.
