Consent to the processing of personal data

With this consent, freely, of my own free will and in my own interest, I give the consent of EF GEF LLC (TIN 7801737783 / OGRN 1247800113120 / KPP 780101001 Address: 199406, City of St. Petersburg, ext. Municipal Okrug Gavan, Maly V.O. Ave., building 64, building 1, building 1, room 100-N) (hereinafter referred to as the “Operator”) to process my personal data using and without the use of automation tools for the purposes, scope and procedure provided for by this Consent and the Personal Data Processing Policy posted on the website https://fgfmed.ru

1. List of personal data:

1.1. last name, first name, patronymic (if available);

1.2. telephone number;

1.3. email address;

1.4. information contained in the text of the request/application (if the User indicates it independently);

1.5. technical and anonymized data about visiting the site: IP address, date and time of access, information about the browser and device (type/model, screen resolution, OS), session identifiers, data about actions on pages (clicks, page views, filling out fields), referral source, UTM parameters, cookies and similar technologies;

1.6. identifiers and data generated by the Yandex.Metrica web analytics service.

2. Purposes of processing personal data:

2.1. processing requests and applications (including making appointments), feedback;

2.2. ensuring the functioning and security of the site, preventing abuse;

2.3. maintaining traffic statistics and improving the quality of the site (anonymized data, cookies) using the Yandex.Metrica web analytics service - in the manner and under the conditions specified in the Cookie Policy.

3. List of actions aimed at processing personal data:

3.1. The operator has the right to carry out the following actions (operations) with personal data: collection, booking, systematization, accumulation, storage, clarification (updating, changing), extraction, use, depersonalization, blocking, deletion, destruction, as well as transfer (provision, access) in cases and on the grounds provided for by the legislation of the Russian Federation and this Consent.

4. Transfer of personal data and processing on behalf of:

4.1. The Operator has the right to entrust the processing of personal data to persons ensuring the functioning of the site and information systems of the Operator (for example, hosting provider, contractors for technical support of the site, web analytics service Yandex.Metrica), subject to the conclusion of appropriate agreements and compliance by such persons with the requirements of the legislation on personal data.

5. Cross-border transfer of personal data is not carried out by the Operator.

6. Duration of consent and processing time:

6.1. This Consent is valid from the moment of putting a mark (tick) in the “I agree to the processing of personal data” field on the website and/or clicking the button to submit the form, as well as other actions that clearly indicate consent (for example, sending a request by e-mail), and is valid until the purposes of processing are achieved or until the consent is withdrawn, unless otherwise provided by the legislation of the Russian Federation;

6.2. Personal data related to inquiries/applications are generally stored for no more than one (1) year from the date of last interaction, unless a longer period is required for the performance of a contract or the fulfillment of legal obligations.

6.3. The storage periods for cookies and technical data are determined by the Cookie Policy and the settings of the User’s browser/device.

7. Procedure for revoking consent:

7.1. Consent can be revoked at any time by sending a notification to the Operator to the email address hello@fgfmed.ru with the note “Withdrawal of consent to the processing of personal data.”

7.2. Upon withdrawal of consent, the Operator stops processing and ensures the destruction of personal data within up to 30 (thirty) days, unless there are other legal grounds for their processing.

8. Confirmations of the subject of personal data:

8.1. I have read the Personal Data Processing Policy posted on the website https://fgfmed.ru

8.2. I am notified that the provision of special categories of personal data (including information about health status) through the site forms is not required; if such information is provided by me on my own initiative, it is processed only to consider the request/record and feedback;

8.3. I was explained the rights of the subject of personal data provided for by Federal Law No. 152, including the right to withdraw consent.

8.4. sending advertising and informational mailings (marketing) is carried out only with a separate consent to online mailing and is not included in this Consent.
